1. Good Morning/Afternoon/Evening Sir/Mam, my name is_____________from _________ Church in _________________. We are taking a survey in your neighborhood this morning/afternoon/evening and would like to ask you just a FEW SPIRITUAL QUESTIONS.
   1. Do you have a church you're attending here in this area Mr./Mrs.____________?
   2. Is the Bible taught/preached in your church?
   3. Do you believe that you must be 'born again' to go the heaven? If they say no, ask them: 'Did you know that Jesus told a religious leader of the Jews that he had to be born again to enter into the Kingdom of God?' (John 3:5) If they say 'yes' ask them if they have ever been born again. Even if they say yes be sure to go on to question D.
   4. I would also like to ask you this VERY IMPORTANT SPIRITUAL QUESTION that we are asking each of your neighbors today: 'If you were to die today, do you KNOW FOR SURE that you would go to heaven to be with the Lord Jesus?'

2. The Bible says in I John 5:13 'These things have I written unto you that believe on the name of the Son of God; that ye may KNOW that ye have eternal life...' So, you see, YOU CAN AND SHOULD KNOW, while you are here on earth, where you are going to spend eternity!

3. Please, let me share with you briefly HOW I CAME TO KNOW for sure I am going to heaven? I was saved/became a Christian/was born again_________years ago when I learned that God loved me even though I sinned against Him. (Give Brief Testimony Including Past/Present/ Future Working/Blessing of God: How you were saved from the penalty of sin/are being saved from the power of sin/ will be saved from the presence of sin for eternity) The Bible teaches that 'All have sinned and come short of the glory of God'. (Romans 3:23)

4. Yes, WE ARE ALL SINNERS because sin entered the world through the first man God created, Adam, and we inherited his sin nature when we were born just as all men have since the beginning. (Romans 5:12) Demonstrate the sins of a child: lying, disobedience to parents, anger, etc. Sins that all of us have committed/and are guilty not having received the forgiveness of God.

5. The Bible clearly tells us that 'The wages of sin is DEATH'. (Romans 6:23) Death, physical and spiritual death which is eternal separation from God in the torment/suffering of hell, is the penalty for sinning against God! That's the bad news, but please listen carefully to the good news because this is the main reason we are visiting you this morning/afternoon/evening!

6. You see, God LOVED us so much that even 'while we were yet sinners' the Lord Jesus Christ shed His Blood and died on the Cross to pay the penalty for our sin. (Romans 5:8) 'For God so loved the world that He gave His only begotten Son, that whosoever believeth in Him should not perish but have everlasting life' (John 3:16). Jesus died and completely paid the penalty for your sin and mine, He was buried and rose again (I Corinthians 15:3,4) victoriously guaranteeing eternal life for all who will believe and receive Him as their Savior!! (John 1:12)

7. Please, let me finish by asking you these LAST THREE QUESTIONS:
   1. Do you believe that you have sinned against God?
   2. Do you want forgiveness from God for your sin?
   3. Do you believe Jesus died to pay for your sin? The Bible says there must be 'Repentance toward God and faith toward Jesus Christ' (Acts 20:21) Repentance simply means you are willing to turn from your sinful way and turn unto Jesus by faith! 'If you will confess with your mouth the Lord Jesus and believe in your heart God raised Him from the dead' (Romans 10:9) you will be saved!

8. For whosoever shall CALL upon the name of the Lord shall be saved' (Romans 10:13) If you want to be saved you must simply call on the Lord Jesus and He will save you now!! 'Now is the accepted time, now is the day of salvation' (II Cor 6:2) Let us bow our heads right now and pray, and if you truly want Jesus to save you from your sin, PRAY AND ASK Him to do what He promised He would do in His Word.

Dear Lord Jesus,

I know that I am a sinner, and I cannot save myself, I believe You died on the Cross for me and rose again to pay for my sins and I ask you to forgive my sins and give me eternal life. I receive you now as my Savior into my heart and life. Thank you for saving me, please help me to live for You and tell others about You and what You have done for me. Amen
